Liability Release & Acknowledgment

By registering for activities at Chappapeela Crafts, I acknowledge that participation in art, craft, blacksmithing and maker activities may involve ordinary risks, including the use of tools, equipment, art materials, and other hands-on activities.

 

I understand that while reasonable safety precautions and supervision will be provided, accidents and injuries can occur.

 

I voluntarily agree to participate and assume responsibility for the ordinary risks associated with participation. I release and hold harmless Chappapeela Crafts, its owners, instructors, assistants, and volunteers from claims arising from my participation, except to the extent caused by gross negligence or intentional misconduct.

 

If registering on behalf of a minor: I confirm that I am the child's parent or legal guardian, have authority to provide this consent, and agree that the above release applies to my child's participation as well.
